What we collect
We collect personal information necessary to provide you with healthcare — including your name, date of birth, contact details, Medicare and concession details, medical history, family history, current medications, allergies and clinical notes from consultations.
How we use it
Your information is used to provide medical care, communicate with treating specialists and allied health providers, claim Medicare and DVA rebates, follow up on results, and meet our legal obligations.
Disclosure
We do not disclose your information to third parties except where required for your care (e.g. to specialists, pathology, hospitals, allied health) or where required by law.
Storage and security
Records are stored electronically with appropriate technical and physical safeguards. Access is restricted to authorised practice staff.
Access and corrections
You may request access to your health record or ask us to correct information you believe is inaccurate. Please contact reception to make a request.
Concerns
If you have concerns about how we have handled your information, please raise them with the Practice Manager. Unresolved concerns may be referred to the Office of the Australian Information Commissioner (oaic.gov.au).
